| In order to end homelessness in Pasadena, the "City of Pasadena 10-Year Strategy to End Homelessness" will focus on three (3) groups of persons (made up of families and individuals) who must receive supportive services (i.e., case management, health care, income assistance, etc.) and be able to obtain and remain in affordable housing. The three (3) groups of persons include: GROUP ONE: Families and Individuals who are presently housed but are at-risk-to-homelessness; GROUP TWO: Families and Individuals who are 1) living on the streets, 2) in shelters and transitional housing programs, and 3) discharged from public and private systems of care such as health care facilities, foster care or other youth facilities, and corrections programs and institutions; GROUP THREE: Families and Individuals who exit 1) life on the streets, 2) from shelters and transitional housing programs, and 3) from public and private systems of care and obtain permanent affordable housing but remain at-risk-to-homelessness |
